Kinesiological study performed Frenchay Arm Test in patients with spastic hemiparesis

This bachelor thesis is devoted to a kinesiological study of the Frenchay Arm Test in patients with spastic hemiparesis. It is a study focused on the kinesiological analysis of selected items in modified Frenchay Arm Test, which is supplemented with mapping of electrical activity of selected muscles by surface electromyography in patients with spastic hemiparesis before and after botulinum toxin application. The theoretical part describes the different manifestations and forms of spastic paresis. Clinical scales for the assessment of spasticity and treatment options are also presented. The last part is devoted to grip functions and selected tests assessing functional abilities of the hand. One of those listed is the Frenchay Arm Test and its modified version. It is a tool that assesses common activities of daily living. In the practical part, a qualitative research was used. The research sample consisted of two probands with spastic hemiparesis who were indicated for botulinum toxin treatment. Kinesiological analysis was performed on each before and after botulinum toxin application. It included medical history, aspect, palpation, examination of sensation, reflexes and muscle strength, goniometry, spasticity examination according to Gracies and modified Frenchay Arm Test (mFAT). Three items were selected from this test, kinesiologically described in the practical section, in which the activity of selected muscles was mapped by surface electromyography (SEMG). By comparing the results, it was possible to observe the changes in muscle activity that occurred as a result of rehabilitation, botulinum toxin application and the kinesiological performance itself. It has been documented that mFAT is a suitable test for monitoring the effects of therapy in patients with spastic hemiparesis. However, it is important that it is evaluated by experienced therapists and its accurate performance is maintained. SEMG is a suitable method for mapping muscle activity, although the results and their interpretation are limited by the methodology of performance and the processing of the recording.
Changes in selected postural stability tests in hemiparetic patients after botulinum toxin application

The subject of this bachelor thesis is to compare changes in selected clinical and posturographic tests of postural stability, before and after application of botulinum toxin A to selected lower limb muscles in patients with spastic hemiparesis. The theoretical part deals with the introduction of general information related to posture, such as clinical tests of postural stability, their significance and use, as well as the issue of falls. The next part concerns spastic paresis, its origin, causes, consequences and also clinical scales that evaluate spasticity. The last theoretical part summarize the treatment options of spastic paresis. The practical part describes in detail the kinesiological analyzes of patients and the results of clinical and posturographic tests. Functional Reach Test, Timed Up and Go and Mini BESTest were selected from the clinical trials. Within the posturography, the Weight Bearing Squat test, Modified CTSIB and Limits of Stability were selected. The overall results of this research were not entirely clear, as the individual tests focus on different aspects of postural stability. Some tests were better after botulinum toxin application and some of them before botulinum toxin application. However, impairment of postural stability was in relation to the hemiparetic half of the body. Research has also shown that vision is a key aspect of postural stability in hemiparetic patients. This research could serve as inspiration in the treatment of postural stability not only in hemiparetics, but also in stability disorders in general.
Influence of Botulinum Toxin Application on Quality of Life in Patients with Multiple Sclerosis

The aim of the bachelor's thesis is to find out how medical application of botulinum toxin can affect the quality of life in patients with multiple sclerosis. The thesis is based on theoretical research providing a brief overview of the basic facts about multiple sclerosis - its pathogenesis, etiology, clinical picture, treatment alternatives. Further on, the thesis deals with the concept of spasticity, possibilities to influence it and also its valuation. The subsequent section comprises an overview of the findings about botulinum toxin and its therapeutic application. The term quality of life, an influence of the disease on the quality of life and also the methods used to evaluate it are described at the end of the theoretical part. The practical section contains case histories of two patients who underwent the medical application of botulinum toxin. The thesis is closed by a discussion on this topic. Effect of botulinum toxin use in muscle spasticity

This study has brought an overview of botulinum toxin and its influence on the human muscles, especially on spastic muscles. In the practical part is resumed experience with botulinum toxin A therapy in children with cerebral palsy. There were observed two groups. Group A (n=9) was measured before and after therapy. Title: Therapy of spasticity with botulinum toxin and assessment of the effect by Tardieu Scale Abstract: This paper discusses paralysing effect of botulinum toxin which is used to treat spastic muscles in contemporary medicine. The paper focuses on spasticity in patients with disorder of the central nervous system caused most commonly by stroke. At first it deals with theoretical knowledge about physiology of movement, then describes active ingredient and explains the mechanism of its effect. It dedicates a lot of attention to follow-up rehabilitation as an integral part in the management of spasticity. This paper is based on two case histories. A practical part describes in detail patient's condition before and after application of botulinum toxin. A progress of the effect was assessed during periodic meetings. These instruments were used to assess: Tardieu Scale of spasticity and Frenchay arm test. In connection with the application of botulinum toxin is emphasized active co-operation of the patient. It is an essential precondition for positive results in the management of spasticity. The effect of prolonged stretching on spastic muscle contracture before and after Botulinum toxin A application

A large proportion of patients with central motoneuron lesions is at risk of spastic paresis and formation of contractures. One of the therapeutic techniques used in patients with spastic paresis is prolonged stretching. The theoretical part summarizes the knowledge regarding spastic paresis, its clinical evaluation, the development of contractures and therapeutic interventions, especially botulinum toxin (BTX) and stretching. The aim of this study was to compare the effectiveness of prolonged stretching on spastic muscle contracture before and after BTX application. In our retrospective comparative study there were included 30 patients divided into two groups. The first group consisted of 15 patients performing prolonged stretching of the m. rectus femoris (m. RF) for three months according to Guided Self-rehabilitation Contract of Professor J. M. Gracies. The second group (15 patients) performing the same procedure, but at the same time BTX was injected into the m. RF. We monitored the change in passive knee joint range of motion into flexion and the change in 10MWT time. The goal of this bachelor thesis is to explore the total number of patients that were treated in center of spasticity at Department of rehabilitation and sports medicine University Hospital Motol in 2013-2017 and access their clinical profile. The theoretical part is dedicated to spasticity, to clinical scales that assess spasticity and to therapy of spasticity. The practical part sum up the avarage dosage of botulinum toxin into a muscle, the most common applied muscles and the subjective efect of the therapy. This part also contains two casuistics. All the results are summarized in tables and graphs.

This Master's thesis focuses on Occupational therapy for patients after application of botulinum toxin to upper limb. The aim of the thesis was to evaluate and compare a progress of functional independence in patients after acquired brain injury. There were twenty patients involved in the research, who were further divided into two groups and separated by ten people in each group. Both groups underwent the application of botulinum toxin whereas the experimental group A went through the occupational therapy. The control group had ordinary rehabilitation at home and without any occupational intervention for the duration of twelve week under guided self-rehabilitation contract of spastic paresis, so-called GSC. A programme of the group A included over the counter rehabilitation outpatient occupational therapy. The patients attended the course twice a week for the duration of four weeks. Afterwards, they received a home programme for the duration of eight weeks, during which they had two examinations at occupational therapy. All the participants were evaluated by Global Subjective Self-Assessment which is focused on spasticity. Title: Effect of application of botulotoxin for change of gait in patients after brain damage with spastic dystonia Abstract: The theme of this bachelor thesis is verifying possibilities of botulinum toxin influencing the gait stereotype of patients with spastic dystonia. The thesis consists of theoretical and practical parts. The theoretical part consists of research of relevant literature sources, based on which basic terms of the associated range of problems are briefly defined: muscle tone, spasticity, spastic dystonia, botulinum toxin and gait. Part of the theoretical summary also includes scales that describe muscle tone (Ashworth scale, Tardieu scale, Five-step clinical assessment by Gracies) and also tests evaluating gait stereotype (Tinetti Gait and Balance Examination, 10 Meter Walk Test, 2 Minute Walk Test, Timed Up and Go, Rivermead visual assessment of the gait). Using the Frenchay Arm Test in monitoring the effect of botulinum toxin in spastic patients after brain damage

bachelor's thesis: Bachelor's thesis deals with the use of Frenchay Arm Test to study the effect of botulinum toxin to patients with spasticity after brain damage. A question is raised, whether Frenchay Arm Test, is test of self-sufficiency and hand function, is useful for monitoring the effects of botulinum toxin in these patients. The theoretical part explains the physiology of human movement and the consequences of brain damage to motoric. It deals with the increased stretch muscle, i.e. spasticity of patients after brain damage. Furthermore, it introduces the theory of application of botulinum toxin, which in small amounts affects spasticity and helps promote self-sufficiency of a patient. Frenchay Arm Test is presented as appropriate diagnostic method and benefits of applying this method by Occupational therapist are described. In the practical part, the Frenchay Arm Test is applied to two patients prior to application of botulinum toxin, month after and three months after application. The results are evaluated based on modified Frenchay scale and complemented by labels marking individual acts.
